NEW STUDENT ENROLLMENT

Please use the form below to request enrollment.

Enrollment is on a first-come, first-served basis, space permitting. You can email us at school@uwms.org if you have additional questions.

Please submit one form per child and include the school grade they'll be starting in September.

Children must be 4 years old by December 31 of the school year to enroll.
